  2. Form Submission
    Fire Hydrant Flow Test Requests may be submitted electronically by completing the information requested below. Alternatively, a written request may be mailed or faxed. Requester will need to provide the location of the hydrant to be tested, including the hydrant number if possible, as well as a map/site plan of the fire hydrant location.
  3. Fees
    If there is a valid fire flow test on file (fire flow tests are valid for one (1) year from the test date), a certified copy may be obtained for a fee of $35.00. If a new flow test is needed, there is a fee of $250.00. The fee must be paid before the test will be conducted. Payment may be in the form of cash, check or credit card. Checks should be made payable to Treasurer of Frederick County and may be mailed or delivered to 4520 Metropolitan Court, Frederick, MD. Credit card payments will incur a 2% fee and may be made over the phone or in person at the above address.
  4. Map/Site Plan
    A map/site plan of the fire hydrant location must accompany the request. That document can be uploaded to accompany this electronic submission or, alternatively, it can be submitted by mail or fax to the address and fax number shown below.
  5. Mailed or Faxed Submissions
    Written requests for fire hydrant flow tests and/or submission of the map/site plan may be mailed or faxed to: Division of Water and Sewer Utilities, 4520 Metropolitan Court, Frederick, MD 21704; fax number 301-600-2349.
  6. Test Results
    A work order will be submitted once the electronic or written request has been received along with the map/site plan of the fire hydrant location. Test results should be available within 10 to 15 business days. Please indicate below the method by which you would like to receive your test results.
