The Community Partnership Grant program is underway! If your agency has been awarded funds, please refer to the information below for report deadlines. This page will be used to update information about the grant program during the award period.
Each CPG grantee awarded One-Time Grants for January 1, 2016 – June 30, 2016 will be required to submit receipts for each purchase, along with a Program Narrative explaining how the purchase(s) directly served the target population by July 29, 2016.
Each CPG grantee awarded Operating Grants for January 1, 2016 – June 30, 2017 will be required to submit Program Assessment and Financial Reports by the following deadlines:
FY 16 Operating Grants Program and Financial reports are due on July 29, 2016
FY 17 Operating Grants Program and Financial reports are due on both January 27, 2017 and July 28, 2017
