The Senior Care Program is designed to help frail, health impaired, older persons locate and select the services that can help them continue to live in their homes and stay in the community.
An evaluation of needs is done in the home by a qualified staff person or team. After the needed services are identified, a case manager will help locate and arrange for the services and ensure the individual receives the needed services at the right time. Such persons may be eligible for gap filling service dollars if they meet established financial standards and other eligibility criteria. There is no cost for the individual.
The Personal Care Program provides in-home aide services to individuals 60 years or older who require assistance with activities of daily living such as bathing, dressing, and meal preparation. In-home aides may also provide chore or homemaker services, as well.
