Qualifications
Must be:
- Caregiver of a child under 21 OR Unmarried, younger than 21, and without caretaker
- Maryland Resident
- Meet the below income requirements:
- Please Note: Parents of children enrolled in MCHP may be eligible, but under these different income requirements
- US Citizen, permanent resident, or Refugee / asylee

How to Apply
- Print an MCHP Application Form

- Call toll-free at (800) 456-8900 or (301) 600-8888 and ask for a MCHP application form
- Apply online at www.marylandsail.org
- Make sure to put your phone number on your form so someone can contact you if they have questions.
- Mail in your completed application form to:

Information Needed to Apply
- Childcare expenses, if applicable
- List other health insurance coverage
- Names, birth dates, social security numbers of family members (guardians and their dependents)
- Proof of US citizenship or residence (Birth certificate; passport; naturalization papers; affidavits)
- Self employed individuals need recent tax returns and schedules
- Sources and amount of income
- State-issued photo identity card (Driver's license or MVA ID)
