Dear Health Care Provider:
The Frederick County Child Health Partnership’s mission is to encourage and enable children of Frederick County to adopt and maintain healthy lifestyles. With this in mind, the Partnership recently received a small grant to support local physicians as they educate families about healthy living.
This childhood obesity toolkit contains sample tools and resources for families. MaineHealth, in collaboration with the MMC Physician-Hospital Organization and the Maine Youth Overweight Collaborative, developed this toolkit as part of the initiative Let’s Go! Eat Right. Be Active. Get Healthy. The toolkit encourages families to practice the following basic health tips:
The toolkit includes brochures, prescription for health, and a 5,2,1,0 poster. Funding for this toolkit was made possible by a grant from State Farm.
