When you attend your appointment we need you to bring:
- All family members applying for WIC:
- Identification for all participants - one of the following:
- Birth certificate
- Driver's license
- Social security card
- The crib card from the hospital for new babies
- Proof of your household income:
- Food Supplement Program (FSP) card - formally food stamps card
- Medical assistance cards for all family members
- Recent family paystubs,
- Proof of your address - one of the following:
- Copy of a lease
- Driver's license
- Official mail
- Proof of pregnancy if applicable - one of the following:
- Doctor's note
- Pregnancy test results
- Sonogram
