The Senior Care program is designed to help frail, health impaired, older persons locate and select the services that can help them continue to live in their homes and stay in the community.
Department of Social Services Senior Care
An evaluation of needs is done in the home by a qualified staff person or team.
After the needed services are identified, a case manager will help locate and arrange for the services and ensure the individual receives the needed services at the right time.
65 years of age or older
Be medically at risk of institutionalization
Be moderately or severely disabled
Live in Frederick County
Need assistance with several daily activities such as bathing, meal preparation, etc.
Such persons may be eligible for gap filling service dollars if they meet established financial standards and other eligibility criteria. There is no cost.
