Frederick County Health Access Program (FCHAP) Overview
- FCHAP connects low-income uninsured Frederick County residents with primary and specialty care providers and provides care coordination services for enrollees.
- This program was initiated by the Frederick County Health Department in partnership with the Frederick County Health Care Coalition.
- The program is supported by grants from Kaiser Permanente of the Mid-Atlantic States and the Community Foundation of Frederick County's Joseph D. Byrd Fund.
FCHAP Providers
Who can apply?
- Frederick County residents, infants through adults, with limited family incomes who have no health insurance and are not eligible for public insurance programs (Medical Assistance, MCHP, etc.).

What are the benefits of FCHAP?
The program provides enrollees with assistance in obtaining care, which includes:
- Assistance with obtaining prescription medicine
- Interpretation services as needed
- Limited labs, x-rays and medical diagnostic tests
- Referrals to community resource
- Transportation assistance
- Visits with a participating primary care provider or specialist for a $15 visit fee

- The Frederick County Health Care Coalition Fund with the Community Foundation of Frederick County accepts tax-deductible donations for the purpose of improving the availability and accessibility of quality health care in Frederick County.
