Employees may select coverage from one of two dental plan options offered through United Concordia. The completed application must be submitted to the Division of Human Resources within 30 days from the employee's first date of employment or benefit eligibility. Frederick County pays $5 of the monthly premiums for dental coverage.

Dental PPO Plan
In the Dental PPO Plan, members may use either in-network or out-of-network dental providers. If the employee chooses a participating dentist from the Advantage Plus network, his/her preventive care (exams, cleanings, and x-rays twice a year) is covered at 100%. For basic services, such as fillings, the employee pays 20%, and for other major services, including orthodontia, the employee pays 50% of a negotiated fee the dentist has agreed to charge for the service. There is no deductible; there are no claims to submit. If the member sees a non-participating dentist, his/her preventive services are still covered at 100% of maximum allowable charges. For basic services, such as fillings, the plan pays 80%, and for other services, including orthodontia, the plan pays 50% of maximum allowable charges, after meeting a $25/individual or $75/family deductible. The member is responsible for paying the out-of-network provider any difference between the fee for the service and the maximum allowable charge. There is a calendar year benefit maximum of $2,000 per member, and an orthodontia lifetime maximum of $1,800 per member. Preventive care services are not included when calculating the maximum benefit.

Dental HMO Plan
The Dental HMO Plan (DHMO), offers dental care from participating providers in United Concordia's DHMO-Concordia Plus network. The member selects a primary-care dentist from the network, and goes to that provider to receive dental care. If specialty services are needed, the member is referred by their primary-care dentist to a specialist in the network. Every time services are received, the member pays a copay, as shown on the schedule of benefits. There are no claim forms to complete and no limit to the services covered in a year. Services from out-of-network dentists are not covered in this plan.
