Home Visitation
Home visiting services are provided to families who are unable to attend the Center for variety of reasons with the hope that each family can be transitioned to participate in center activities, as appropriate. These services are provided to expectants and / or families with children 0 to 3 years old. Family Partnership also provides limited home visiting to families with children 4 - 5 years old.

Home visiting services can include the following:
  • Employment Services - Helping participants explore career options and develop systems for household management, childcare, and emergency plans for sick child and transportation.
  • Health Education Services - Encouraging and empowering participants to access medical care for themselves and their children to improve parent and child health. Information, education and referrals regarding family planning and general prenatal health are provided. Parents are encourage and assisted to keep well baby appointments and maintain immunizations records. Home visitors can assist parents in evaluating their house using home safety guidelines and encourage families to maintain home safety needs.
  • Infant / Toddler Services - Developmental assessment, home safety information, and immunization monitoring.
  • Mental Health Services - Staff provide support and appropriate referrals to other resources.
  • Parent Education Services - Assisting parents in gaining and enhancing parenting skills through the use of Parents as Teachers curriculum and / or other parent education resources.
  • Self-Sufficiency Services - Assisting participants in locating and accessing education programs, tutoring programs, and assisting families in finding adequate child-care.
  • Social Support Services - Staff encourages the participants to attend FP and center programming and assist participants to establish linkages with community resources or friends and family.
